Our certified Wound & Ostomy Nurse uses evidence based practice to provide quality health care to clients with wounds and ostomy disorders. Our CWOCN experience combines for a complete plan of care.

 

 

Certified Skilled Pediatric Services

Our nurses provide private duty one on one care for children requiring highly specialized treatments at home and while at school. Children with developmental and congenital disorders require a case management approach to meet their challenging health needs. We provide comprehensive assessments and regular status reports to the pediatrician and other specialists. The family is the focus of care while providing for the respite needs of highly compromised children.

Our professional, competent, and compassionate staff provide home care services for patients who are technology dependent including, but not limited to, the use of:

 

Enteral feedings

Invasive mechanical ventilation

Multiple, daily respiratory treatments

Apnea monitoring services

CPAP therapy

Diagnostic testing (pulse oximetry, lab draws)

Oxygen therapy

Respiratory disease management

Services are available for infants and children who have chronic conditions that require the ongoing care and management by a professional nurse and therapists in the home and school environment.

 

 

Honeywell HomMed is a home telemonitoring program that uses state-of-the art technology to monitor the health status of an individual at home.

The monitoring equipment is placed in the home to accurately measure a patient’s blood pressure, weight, pulse, and pulse oximetry (blood oxygen levels) daily. These readings are transmitted daily over the phone lines to a central monitoring station at the home health agency.

The monitor is also programmed to ask up to 10 specific questions customized to the patient’s particular condition. This information helps the nurse further understand the vital signs information that is received at the agency’s central station.

The results are closely monitored by a registered nurse. The nurse reviews the data and patients are called when appropriate. The home health nurse communicates with the patient’s physician as needed in addition to providing bi-weekly reports to the patient’s physician on the patient’s vital signs.

This system is available to all patients while receiving skilled services. Patients may elect to continue to use the service for a monthly service fee after discharge from skilled services.
